You will first need to add the provider in Configuration -> Servers. You will find it under Mail Filters and we normally recommend adding to this to the server running the mail. You will then need to add to the virtual server (If you use one) and hosting plan.

Once it is enabled in the hosting plan then domains are automatically created based upon the defaults set in mailcleaner.
