I have set database prefix as '[User_Name]_' from 'Policies' option
But when add database it show only '[User_Name]' and missed the UNDERSCORE (_) from the end
Same issue with database username
Same for MySQL
Any help?

Hello,
I have set a prefix of: [USER_NAME]_
It appeared to work as expected and the user and database name have this set on both MsSQL and MySQL.
Can you confirm which versions of MySQL and MsSQL you are using?
