i have running a split DNS and access the mail server with the same internal / external name  ( Create a DNS Record to resolve the external as intertnal name )
I have only problems when i create a mailbox, no issue with creating users or some other.

Solved the problem now self.

It helps following :

Internal URL must match the internal Exchange url and not the external one, that fix the problem.

For example External : https://mydomain.com/Powershell
and for internal : https:SRV-MX.mydomain.local/Powershell

That is correct, for external url to work you need to have external powershell priveledges (which atleast back when i was testing it was still bugged even if it was set exchange still provided access denied on remote urls).

It's why our guide recommends using the local fqdn of the server instead of any common url / cluster url etc.

Hello Marco, did I miss something? Since when do you have a manual? I think we've been waiting for years 😛
Unfortunately, it is so in your online video tutorials on much is not received and that it comes across wrong. In the Exchange video z.b. external url equal to internal url, public folder, etc.
This leads in the forum again and again to queries and confusions.
Ev. It would be helpful to make a new video and then also to address the issues. A little explanation would be nice and please turn off the horrible music in the videos.
Music in any video tutorials are absolutely unnecessary and annoy me again and again.
