If you go to the Hosted Org in Configuration -> Servers and change the username Format does this solve your issue?

We have a similar issue, but for us it is "Prepend OrgID" that doesn't work. Our usernames are created such as username0000. Our DB and Portal settings look correct. Version 1.4.8 on all components. We have tried switching the username format to Append OrgID which works as expected but switching it back to Prepend looks the same as when we set it to Standard Username Format.
